What You’ll Do as a Care Manager (RN)

Depending on the program fit, you will:

• Coordinate care for individuals with mental health or substance use conditions
• Link clients to vital services like housing, medical care, therapy, and social support
• Complete assessments that determine eligibility for community-based services
• Advocate for clients’ needs across behavioral health, medical, and social systems

Who We’re Looking For

We’re hiring RNs with bachelor’s degrees (or equivalent experience) who are:

✅ Organized and detail-oriented

✅ Comfortable working in the field and across systems

✅ Ready to take the next step in their nursing career

Open RN Roles Include:

Health Home Plus (HH+) Care Manager

• Work with adults with serious mental illness or substance use disorders
• Requires RN + 2 years’ experience in behavioral health (Master’s may count for 2 years)

AOT (Assisted Outpatient Treatment) Care Manager

• Support individuals under court-ordered treatment plans
• Requires RN + 4 years of experience (or Master’s + 2 years)

HCBS/HARP Assessor

• Conduct behavioral health assessments for state service eligibility
• Requires RN + training in UAS/HCBS assessment tools

HIV/AIDS Program Care Manager

• Coordinate comprehensive care for clients with HIV/AIDS
• Requires RN + 2 years’ experience (or Associate’s RN with 2 years)
