Health Home Care Manager – Full-Time (New York State)

Doral Health and Wellness is a leading multi-specialty healthcare organization serving diverse communities across New York State. Our mission is to deliver integrated, compassionate, and patient-centered care, especially for individuals managing chronic medical, behavioral health, and developmental conditions. As a New York State-designated Health Home provider, we are expanding our Care Management team to better serve vulnerable Medicaid populations.

Position OverviewWe are seeking a dedicated Health Home Care Manager to coordinate person-centered services for Medicaid-enrolled clients with chronic medical, mental health, or substance use challenges. This hybrid position combines remote work with field and community visits within New York City, including in-home visits across Brooklyn and surrounding areas.

Key Responsibilities

  • Conduct initial and ongoing assessments to create and update person-centered care plans
  • Coordinate services across physical health, behavioral health, substance use, and social service providers
  • Assist clients in navigating healthcare systems, scheduling appointments, securing housing, and accessing benefits
  • Promote self-management of medical and behavioral health conditions
  • Maintain accurate and timely documentation in electronic health record (EHR) systems
  • Comply with New York State Health Home guidelines and quality standards
  • Collaborate with families, care teams, community resources, and providers to support clients well-being

Qualifications (Required by NYS DOH)Candidates must meet one of the following educational and experience requirements:

  • Bachelors degree in a health or human services field plus 2 years of qualifying experience
  • Masters degree in a health or human services field plus 1 year of experience
  • Licensed Registered Nurse (RN) plus 2 years of relevant experience

Additional Requirements

  • Familiarity with Medicaid populations, chronic conditions, and social determinants of health
  • Strong communication, organizational, and case management skills
  • Comfortable using computers and electronic systems
  • Proficiency with Microsoft Office and EHR platforms
  • Ability to travel for field and community-based visits within New York City
  • Bilingual (Spanish, Haitian Creole, Bengali, or other languages) strongly preferred
