We are seeking a dedicated HCBS Service Coordinator to support individuals receiving Home and Community-Based Services. This role involves coordinating services, ensuring care plans are followed, and helping clients live independently in their communities.
Key Responsibilities:
- Coordinate and monitor HCBS services for assigned clients
- Develop and implement person-centered care plans
- Collaborate with interdisciplinary teams and service providers
- Maintain accurate documentation and compliance with state guidelines
- Advocate for the needs and rights of clients
Qualifications:
- Bachelor’s degree in health, human services, psychology, social work, or related field
- Experience working with individuals with behavioral health needs preferred
- Strong organizational and communication skills
- Familiarity with HCBS or case management is a plus
