We are seeking a qualified Registered Dietitian / Certified Dietitian-Nutritionist (RD/CDN) to join our growing healthcare team in Brooklyn, NY, providing specialized nutrition care to patients undergoing dialysis.

Position Overview

The Registered Dietitian / Certified Dietitian-Nutritionist will provide individualized nutrition assessments, counseling, and dietary guidance to patients receiving dialysis. The goal is to help patients manage their nutritional needs, maintain appropriate dietary habits, and improve their overall health and quality of life.

The ideal candidate will have a strong background in nutrition and dietetics, with experience in renal nutrition or dialysis care preferred. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to support coordinated, patient-centered care.

Key Responsibilities

  • Conduct comprehensive nutrition assessments for patients undergoing dialysis.
  • Develop and implement individualized nutrition care plans based on patients’ medical conditions, laboratory results, and dietary needs.
  • Educate patients on renal diets, including sodium, potassium, phosphorus, protein, and fluid management.
  • Monitor patients’ nutritional status, weight changes, laboratory values, and dietary intake.
  • Provide guidance on meal planning, appropriate food choices, and nutritional supplementation when indicated.
  • Help patients understand the relationship between nutrition, kidney disease, and dialysis treatment.
  • Collaborate with nephrologists, nurses, and the multidisciplinary dialysis care team to coordinate patient care.
  • Monitor patient progress and adjust nutrition interventions as appropriate.
  • Identify nutritional concerns and communicate recommendations to the healthcare team.
  • Provide ongoing education, encouragement, and support to patients and their families.
  • Maintain accurate and timely documentation of nutrition assessments, care plans, interventions, and follow-up visits in the EHR/EMR system.
  • Ensure compliance with applicable clinical standards, organizational policies, and healthcare regulations.
  • Stay current with evidence-based nutrition practices and guidelines related to renal nutrition and dialysis care.

Qualifications

  • Bachelor’s degree or higher in Nutrition, Dietetics, or a related field, consistent with applicable New York State requirements.
  • Registered Dietitian (RD/RDN) credential and active New York State Certified Dietitian-Nutritionist (CDN) certification required.
  • Knowledge of nutrition science, therapeutic diets, nutritional assessment, and individualized nutrition care planning.
  • Experience in renal nutrition, dialysis care, or a related clinical setting preferred.
  • Familiarity with dietary management for patients with chronic kidney disease, including sodium, potassium, phosphorus, protein, and fluid management.
  • Ability to interpret relevant laboratory results and incorporate findings into individualized nutrition care plans.
  • Strong patient education, counseling, and communication skills.
  • Experience documenting nutrition assessments and interventions in EHR/EMR systems.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently while collaborating effectively with a multidisciplinary healthcare team.
