Fee-for-Service RN – Pediatric Community-Based Care (HCBS)

Location: 5 Boroughs of NYC and Westchester County

Employment Type: Fee-for-Service / Flexible

Compensation: $150/hour

Doral Health & Wellness is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality care to medically fragile pediatric patients through our Home and Community-Based Services (HCBS) program.

This role focuses on delivering patient-centered care in home and community settings, with an emphasis on supporting patients and families and improving quality of life.

Key Responsibilities

• Provide comprehensive nursing assessments and ongoing care for medically fragile pediatric patients
• Provide patient-centered care in home and community settings
• Monitor patient conditions and communicate changes in status to the appropriate care team
• Collaborate with patients, families, and interdisciplinary care teams
• Support patients and families with complex medical needs
• Maintain accurate and timely clinical documentation
• Ensure compliance with HCBS and New York State requirements

Requirements

• Active New York State RN license
• Experience working with medically fragile patients
• Pediatric clinical experience is required.
• Home care or community-based experience preferred
• Strong clinical, communication, and documentation skills
• Ability to provide compassionate, patient-centered care in home/community settings
