About the Role

We are seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our Care Management team. The LPN Care Manager will help coordinate patient care, monitor members' needs, communicate with healthcare providers, and connect patients with appropriate healthcare and community resources.

This is an excellent opportunity for an LPN who enjoys building relationships with patients and working collaboratively to improve health outcomes.

Responsibilities

• Coordinate and monitor patient care plans and services
• Conduct patient follow-ups and assess ongoing care needs
• Communicate with patients, families, healthcare providers, and community organizations
• Assist with referrals and connections to appropriate healthcare and community resources
• Monitor patient progress and identify changes in health or care needs
• Follow up on appointments, treatments, and recommended services
• Maintain accurate and timely care management documentation
• Collaborate with nurses, providers, social workers, and other members of the care team
• Advocate for patients and help them navigate healthcare and community resources
• Ensure compliance with applicable healthcare and organizational standards

Qualifications

• Current New York State LPN license in good standing
• Previous nursing experience required
• Experience in care management, case management, home care, Health Home, or community healthcare preferred
• Strong communication and organizational skills
• Excellent documentation and computer skills
• Ability to work independently and as part of a multidisciplinary team
• Compassionate and patient-centered approach
