We are seeking a Certified Diabetes Educator (CDE) to join our growing healthcare team in Brooklyn, NY.

Position Overview

The Certified Diabetes Educator will provide comprehensive diabetes education and support to patients, helping them develop the knowledge and skills needed to effectively manage their condition and improve their overall health and quality of life.

The ideal candidate will have strong experience in diabetes education, nutrition, lifestyle management, and patient counseling, with the ability to work collaboratively with healthcare providers and patients.

Key Responsibilities

  • Provide individualized diabetes education and counseling to patients.
  • Educate patients on diabetes management, healthy lifestyle changes, nutrition, exercise, and medication adherence.
  • Teach patients proper blood glucose monitoring and help them understand and manage their glucose levels.
  • Provide guidance on meal planning, carbohydrate management, weight management, and healthy eating habits.
  • Support patients in developing realistic and sustainable diabetes self-management goals.
  • Monitor patient progress and identify barriers to effective diabetes management.
  • Collect and document relevant patient data, including weight, glucose readings, nutrition, medication adherence, and lifestyle factors.
  • Communicate patient progress and concerns to the appropriate healthcare providers and department leadership.
  • Reinforce treatment plans and recommend appropriate lifestyle or educational interventions within the scope of practice.
  • Provide ongoing encouragement, motivation, and support to patients and their families.
  • Maintain accurate and timely documentation in the EHR/EMR system.
  • Stay current with diabetes education standards, evidence-based practices, nutrition recommendations, and emerging approaches to diabetes management.
  • Participate in a multidisciplinary care team to support coordinated, patient-centered care.
  • Maintain compliance with organizational policies, professional standards, and applicable healthcare regulations.

Qualifications

  • Bachelor's degree or higher in Nutrition/Dietetics, Diabetes Education, Healthcare, or a related field.
  • RD/RDN, CDE, or CDCES credential required.
  • Experience in diabetes care, diabetes education, nutrition, or a related clinical setting.
  • Strong knowledge of diabetes management, nutrition, lifestyle modification, and patient education.
  • Experience providing individualized education and counseling to patients with diabetes.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and patient counseling abilities.
  • Experience using EHR/EMR systems for clinical documentation.
  • Excellent organizational skills and attention to detail.
  • Ability to work independently while collaborating effectively with a multidisciplinary healthcare team.
